Immunizations Examination Record Authorization for Medication |
Dear Parent/Guardian,
Welcome. I hope your experience at St. Augustine’s School will be
as wonderful as mine. It is my pleasure to take care of your
children.
My role as school nurse is to ensure a safe and healthy environment for learning. This goal is best achieved through prevention. Early screening solves many health and learning issues. Each year students will have their vision and hearing tested, height and weight measured, and will be screened for scoliosis. These steps lead to early prevention or intervention for potentially more serious problems. New York State requires several immunizations prior to school entry to prevent the spread of communicable diseases. It is my job to be sure each child at St. Augustine’s School has been immunized for the benefit of all the St. Augustine’s School students and their families. The following is a list of all immunizations required to attend St. Augustine’s School. Any new or returning student must show proof of immunization for the grade to be entered. Proof of immunization includes a copy of a previous school record or immunization record from your physician. Please be advised that your child’s registration will not be official until proof of immunization is provided to St. Augustine’s School. Immunizations Required to Enter Kindergarten
6th Grade Addendum Students who are entering 6th grade on or after September 1, 2007 and who are 11 years of age or older must receive an immunization containing tetanus toxoids, diphtheria, and acellular pertussis (Tdap). If a student has received a Td, DT, or a DTaP vaccination within the last two years, the student's Tdap vaccination should be deferred (with rare exceptions) until a period of two years has elapsed. 10 year old students who are entering 6th grade will not be required to receive a Tdap vaccine and will not be excluded from school, but they must be immunized when they turn 11 years old.
